We would like to thank you for taking part in our new process of accepting athletic participation paper work online. This new automated process will allow us to be more efficient in handling the forms as well as saving valuable resources.

All required paperwork (except the physical exam form) will be done electronically.

• The parent/guardian AND the student athlete must fill out and sign all required spaces. Forms will not be accepted electronically without everything filled out by the student athlete and the parent/guardian.

• To access the online forms hold your curser over the “Electronic Participation Forms” tab. You will get a drop down list of the forms.

• Click on the form name and fill out the information requested. (You must have your students ID number available as it is required on each form)

• To submit forms online, you will be asked to provide information such as home phone numbers, cell phone numbers, email addresses, name of primary physician, insurance policy information, and other pertinent information. Please make sure this information is accurate and up to date as this information will be used in case of an emergency.

• To sign the document click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature”. If you make a mistake and need to start over click on the refresh icon next to the signature box. If you are using an IPAD, you may create an electronic signature by holding your finger to the screen and signing your name or by using a stylus.

• Once you have filled out all of the information on each page you will have the opportunity to print the document.

• Please provide an email address so that you can receive a confirmation email once the document has been reviewed by the LVISD staff.

• UIL Medical History, UIL Forms, Emergency Consent Form, Drug Testing Form, and Release of Medical Information forms are required to be completed every year.

• UIL Physicals are required for all incoming 7th, 9th & 11th grade students.

• UIL Physical may be required based upon answers on the UIL Medical History

• Students are required to use the Pre-participation - Physical Examination Form provided. NO OTHER Physical Examination Form can be accepted. The form can be accessed from the Physical/Medical History tab. You are encouraged to fill out the history portion of the form prior to printing. Forms are also available on all Secondary Campuses. Any physical administered prior to April 1, will not be valid.
